The following links provide an opportunity to download and print forms that pertain to various activities and functions at Gerstell Academy. Medical Forms

Dear Parents or Guardian:

If you have not already done so, please make an appointment with your child’s physician for their school physical. It is an annual requirement at Gerstell and needs to be completed and submitted by the start of the school year.

The following health forms are required for the School Year 2010 - 2011. The first three forms are required for each student. The other forms are as needed if your child meets the requirement described:

• Emergency Information and Medical Release form is required by all students (completed and signed by parent/guardian
• Physical Examination form is required by all students (completed and signed by physician)
• Maryland Immunization Certificate form is required by all students (completed and signed by physician)
• Health Inventory Addendum form - only required by children age 6 years and under (completed and signed by physician)
• Gerstell Medication Policies & Guidelines
• Medication Administration Authorization form - (Please read the letter re: Gerstell Medication Policy and Guidelines) This form is only if your child will take medication at school and includes over-the-counter medications (completed and signed by an authorized prescriber and the parent/guardian).
• Health Care Action Plan for Allergies form only required for students with a known allergy (to be completed and signed by physician).
• Letter explaining the Physician Directed Treatment Protocol for anyone who has an unknown severe allergic reaction at school.

Other Health-related Forms

• Management of Diabetes at School form (required on an as-needed basis only)

Please return required forms by mail or drop them off at the office by Monday, August 2nd, 2010.
