Dear Parents or Guardian:
If you have not already done so, please make an appointment with your child’s physician for their school physical. It is an annual requirement at Gerstell and needs to be completed and submitted by the start of the school year.
The following health forms are required for the School Year 2009-2010:

Emergency Information and Medical Release form (completed and signed by parents)

Physical Examination form (completed and signed by physician)

Health Inventory Addendum form (completed and signed by physician – for children age 6 years and under)

Maryland Health Immunization Certificate form (completed and signed by physician)

Gerstell Medication Policies & Guidelines

Request to Administer Medication at Gerstell - Please read the letter re: Medication Policy and Guidelines. This form must be completed and signed by an authorized prescriber and the parent/guardian only if your child will take medication at school. This includes over-the-counter medications.

Letter explaining the Physician Directed Treatment Protocol to be used for anyone who has an unknown severe allergic reaction at school.

Health Care Action Plan for Allergies form (to be completed and signed by physician only for students with a known allergy)
Please return forms by mail or drop them off at the office by Monday, August 3rd.
